All Posts Going to Moderator Review

Team - I would appreciate a clear understanding of why I keep going into a moderator review status. If something gets posted that goes against the guidelines, I cannot correct it moving forward if I don't know what I have done. As a subscriber, I am getting annoyed by the current process.
